One copy of the Application-Printout duly  completed  in  all  respects, signed and attested by a gazetted officer along with the photocopy of Class10th and 12th Certificates and Mark sheets (attested by a gazetted officer) with superscription  on  the  envelope  “ONLINE APPLICATION 10+2 TES 27.
